Amount due is payable by cheque in the name of “AIM for SEVA”.

WAIVER AND RELEASE AGREEMENT

I/We hereby acknowledge and agree that I/We voluntarily enter this golf tournament at my sole risk, and that AIM FOR SEVA, its affiliates, subsidiaries, associated organizations and the organizers, sponsors and volunteers of this tournament (collectively, the “Tournament Hosts”) are exempt from all liabilities whatsoever for any and all damages, injuries and loss, including personal and property loss, that I may sustain arising from or in connection with any cause whatsoever, including, without limitation, negligence. In consideration of the acceptance of my entry to the golf tournament by the Tournament Hosts,

I/We hereby for myself, my heirs, executors and administrators: (i) waive any and all rights and claim for damages sustained by me as a result of this event for any cause whatsoever, including, without limitation, negligence; and (ii) release the Tournament Hosts from all liabilities associated therewith and indemnify them for same.

I/We warrant that I am physically fit to participate in this event.

I/We hereby recognize that my photograph, electronic image and/or video image may be captured during this event. I give my permission to the Tournament Hosts to use these images through any media at any point in time. I hereby assign and transfer to the Tournament Hosts any and all rights, including copyright that I may now or hereafter have on such images.

I/We acknowledge having read, understood and agreed to the above waiver, release and indemnity.
